Are the sample audio / midi files copyrighted. If so whom does one approach for permission to use? Would appreciate if you could let me know, please. Thank you.

You are feel free to use any samples of copyrighted hymns or any audio files of public domain hymns. There are some audio files connected to products in our store which are designed to give a preview of the product. These files would be copyrighted by the publisher of the product.
