"O Lord Our Lord" Horatio Palmer

Does anyone have a scan of "O Lord, Our Lord (how excellent Thy name)" which was written by Horatio Palmer and first published in "The Song Monarch" in 1874? I see Palmer listed as a composer on hymnary.org, and I see "O Lord Our Lord how excellent Thy name" (I guess it's the same one) listed, without a composer and without any lyrics or page scans. I know of several hymnals with that hymn, but I guess none of them have been scanned.
